PCL Injuries

A posterior cruciate ligament (PCL) injury is a condition that affects one of the major stabilizing ligaments in the knee. The PCL sits deep inside the joint and connects the thighbone to the shinbone. Its primary role is to prevent the shinbone from moving too far backward relative to the thighbone. When the PCL is injured, this stability is compromised, which can affect how the knee moves and bears weight.

PCL injuries are less common than anterior cruciate ligament (ACL) injuries, but they are often more complex. In some cases, the PCL is injured on its own. In others, it may be part of a broader knee injury involving multiple ligaments, cartilage, or bone. These injuries are frequently seen in high-impact situations, including sports collisions and motor vehicle accidents.

What are the symptoms of a PCL injury?

Symptoms of a PCL injury can vary depending on the severity of the damage. Some people notice immediate pain and swelling, while others experience more subtle symptoms that develop over time.

Common symptoms include:

  • Pain in the back of the knee that may worsen with activity
  • Swelling that can occur shortly after the injury or develop gradually
  • A feeling of instability, especially when walking downhill or descending stairs
  • Stiffness and reduced range of motion
  • Difficulty bearing weight on the affected leg

Unlike ACL injuries, a PCL tear may not always produce a dramatic “pop” sensation at the time of injury. Because of this, some individuals underestimate the severity of the problem and delay seeking care. Over time, untreated PCL injuries can lead to abnormal joint mechanics, increasing stress on cartilage and raising the risk of early arthritis.

What are the causes of a PCL injury?

PCL injuries are typically caused by a direct force that pushes the shinbone backward. This mechanism is different from many other knee ligament injuries, which often involve twisting or pivoting motions.

One of the most common causes is a dashboard injury during a car accident. In this situation, the knee is bent, and the shinbone strikes the dashboard, forcing it backward. This direct impact places significant stress on the PCL and can lead to partial or complete tearing.

Sports-related injuries are another major cause. Athletes may injure the PCL during contact sports such as football, soccer, or rugby. A fall onto a bent knee with the foot pointed downward can also drive the shinbone backward and damage the ligament. This type of injury is sometimes seen in sports like basketball or skiing.

Less commonly, PCL injuries can occur due to hyperextension of the knee, where the joint is forced beyond its normal straight position. This can happen during high-speed movements or awkward landings.

In many cases, PCL injuries do not occur in isolation. High-energy trauma can damage multiple structures within the knee, including other ligaments, menisci, and cartilage. These combined injuries are more serious and require a more detailed evaluation and treatment plan.

What are the risk factors?

Certain factors increase the likelihood of sustaining a PCL injury. Participation in high-impact or contact sports places repeated stress on the knee and raises the risk of trauma. Activities that involve sudden stops, changes in direction, or collisions are particularly demanding on the ligaments.

Previous knee injuries can also increase vulnerability. When the knee has already been weakened or altered, it may be less able to withstand force. Muscle weakness, especially in the quadriceps, can contribute to instability and reduce the knee’s ability to absorb impact.

Environmental factors such as slippery surfaces or uneven terrain can also play a role, particularly in sports or outdoor activities. Poor technique or inadequate conditioning may further increase the risk.

How is a PCL injury diagnosed?

Diagnosing a PCL injury requires a careful and systematic approach. The process begins with a detailed medical history. Dr. Faucett will ask the patient to describe how the injury occurred, what symptoms they experienced at the time, and how the knee has felt since.

Then he will perform a physical examination. He will assess the knee for swelling, tenderness, range of motion, and stability. Specific tests are used to evaluate the integrity of the PCL. One of the most important is the posterior drawer test, where the shinbone is gently pushed backward to assess how much movement occurs. Increased backward movement compared to the uninjured knee suggests a PCL injury. Additional tests may be performed to evaluate other structures within the knee. This is important because combined injuries are common and can affect treatment decisions.

Imaging plays a critical role in confirming the diagnosis. X-rays are often obtained first to rule out fractures or bone avulsions, where a piece of bone is pulled away by the ligament. Magnetic resonance imaging (MRI) is the most effective tool for visualizing soft tissues. It provides detailed images of the PCL, surrounding ligaments, cartilage, and menisci.

The severity of the injury is typically graded based on how much instability is present. Mild injuries involve partial stretching of the ligament, while more severe injuries involve complete tears and significant instability. This grading helps guide treatment planning and sets expectations for recovery.

Why early evaluation matters

PCL injuries are sometimes overlooked because symptoms may be less dramatic than other knee injuries. However, the long-term impact can be significant if the condition is not properly addressed. Ongoing instability can alter how the knee functions, leading to increased wear on the joint surfaces.

Early evaluation allows for an accurate diagnosis and a clear understanding of the extent of the injury. It also helps identify any associated damage that may require attention.
