Multiligament Knee Injuries
Multiligament knee injuries, often referred to as MLKI, occur when two or more of the major stabilizing ligaments in the knee are torn or severely damaged at the same time. The knee relies on four primary ligaments to maintain stability and control movement: the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL), and lateral collateral ligament (LCL). When multiple ligaments are injured together, the knee becomes unstable in more than one direction, making this condition significantly more serious than an isolated ligament injury.
These are severe and relatively uncommon injuries. Because several structures are involved, MLKI injuries can affect not only stability but also joint alignment, cartilage health, and surrounding nerves and blood vessels. Prompt evaluation is critical.
Unlike a single ligament tear, which may still allow limited function, a multiligament injury often results in a knee that cannot support weight or maintain normal motion. The complexity of these injuries requires careful diagnosis and a structured treatment plan to restore function and prevent long-term damage.
What are the symptoms of multiligament knee injuries?
The symptoms of a multiligament knee injury are typically immediate and difficult to ignore. Most individuals experience sudden and severe pain at the time of injury, often accompanied by a feeling that the knee has “given out” or shifted out of place. In some cases, the knee may visibly deform or appear dislocated, which is a medical emergency.
Swelling usually develops quickly due to internal bleeding within the joint. This swelling can be significant and may limit motion within hours. Bruising around the knee is also common and may extend down the leg over the following days.
Instability is one of the most defining symptoms. Patients often describe a sense that the knee cannot be trusted to hold their weight. Walking may be extremely difficult or impossible without assistance. Even small movements can feel unstable or unpredictable.
Loss of range of motion is another key feature. Bending or straightening the knee may be limited due to pain, swelling, or mechanical disruption within the joint. Some patients may also experience locking or catching sensations.
In more severe cases, there may be numbness, tingling, or weakness in the lower leg or foot. This can indicate involvement of nearby nerves, particularly the peroneal nerve, which runs along the outer part of the knee. Vascular injury, although less common, is a serious concern and may present as changes in skin color, temperature, or diminished pulses in the foot.
Because these symptoms can overlap with other knee injuries, the severity and combination of symptoms often raise concern for a multiligament injury rather than a single ligament tear.
What are the causes of multiligament knee injuries?
Multiligament knee injuries are typically caused by high-energy trauma or significant force applied to the knee. The most common cause is a direct blow to the knee, especially when the leg is planted and the body is moving in a different direction. This type of force can overwhelm the stabilizing structures of the knee and result in multiple ligament tears.
Motor vehicle accidents are a leading cause, particularly when the knee strikes the dashboard during a collision. This mechanism often results in injury to the PCL along with other ligaments. Falls from height or industrial accidents can also produce similar forces.
In sports, MLKI injuries are usually associated with contact or high-speed movements. Football, soccer, skiing, and basketball are common settings where these injuries occur. A forceful hit to the knee, combined with twisting or hyperextension, can damage multiple ligaments at once.
Non-contact mechanisms are less common but still possible. A sudden change in direction, awkward landing, or hyperextension injury can place extreme stress on the knee, particularly if the muscles are fatigued or coordination is impaired.
Certain factors may increase the risk of sustaining a multiligament injury. These include participation in high-impact sports, poor neuromuscular control, previous knee injuries, and inadequate conditioning. However, many cases occur in otherwise healthy individuals due to the magnitude of the force involved.
How are multiligament knee injuries diagnosed?
Diagnosing a multiligament knee injury requires a thorough and systematic approach. Dr. Faucett will ask for a detailed history of how the injury occurred. Understanding the mechanism of injury provides important clues about which structures may be involved.
He will perform a thorough physical examination to assess swelling, alignment, range of motion, and stability. Specific stress tests are used to evaluate each ligament individually. In the setting of significant pain and swelling, this exam may be limited initially, and further evaluation may be needed once the acute symptoms subside.
Imaging plays a critical role in confirming the diagnosis. X-rays are typically obtained first to evaluate for fractures or joint dislocation. They can also reveal subtle changes in alignment that suggest ligament injury.
Magnetic resonance imaging (MRI) is the most important tool for assessing multiligament injuries. MRI provides detailed visualization of the ligaments, cartilage, menisci, and surrounding soft tissues. It allows the physician to identify which ligaments are torn, the severity of the injury, and whether other structures are involved.
In cases where there is concern for vascular injury, additional imaging such as a CT angiogram may be required. This is especially important if there are signs of reduced blood flow to the lower leg. Early detection of vascular injury is critical to prevent serious complications.
Neurologic evaluation is also essential. The function of the nerves around the knee, particularly the peroneal nerve, must be assessed to identify any deficits that may require further management.
Multiligament knee injuries are complex and require careful coordination of clinical examination and imaging findings. Early and accurate diagnosis allows for appropriate planning and improves the likelihood of restoring stability, function, and long-term joint health.
