Gluteus Medius Injuries
Gluteus medius injuries involve damage to the gluteus medius muscle, most commonly in the form of a strain or tear. What are the symptoms of a gluteus medius injury?
The most common symptom is pain along the outer side of the hip. This pain is typically localized to the area just above the upper thigh and may feel deep or aching in nature. In acute injuries, the pain can be sharp and immediate, especially at the time of the strain or tear.
Pain often worsens with movement, particularly activities that require standing on one leg, climbing stairs, or walking for extended periods. Running, pivoting, and lateral movements can also aggravate symptoms. In milder strains, discomfort may only appear during activity, but in more significant injuries, pain can persist even at rest.
Weakness in the hip is another hallmark symptom. Because the gluteus medius stabilizes the pelvis, injury can lead to difficulty maintaining balance. Patients may notice that their hip feels unstable or that they are unable to control their movement as well as before. This can result in a limp or an altered walking pattern.
Tenderness over the outer hip is common, and pressing on the muscle often reproduces pain. What are the causes of a gluteus medius injury?
A tear of the gluteus medius muscle can occur when a fall or sudden movement forces the hip beyond its normal range of motion, placing excessive stress on the muscle fibers and causing them to tear. These injuries are also common in sports that involve rapid changes in direction, sprinting, or jumping. In some cases, underlying conditions such as tendinopathy, which involves degeneration or inflammation of the surrounding tendon, can weaken the area and increase the likelihood of a muscle tear.
Repetitive use is another major cause. Activities that involve frequent hip motion, such as long-distance running or repetitive side-to-side movements, can gradually fatigue the muscle. When the muscle becomes fatigued, it is less able to absorb force, increasing the risk of injury.
Muscle imbalance plays a significant role. Weakness in the gluteus medius or surrounding muscles can place additional stress on the muscle during activity. If other muscles, such as the hip flexors or lower back muscles, are overactive, they can alter normal movement patterns and increase strain on the gluteus medius.

How is a gluteus medius injury diagnosed?
Diagnosis begins with a detailed understanding of symptoms and how the injury occurred. A history of sudden pain during activity or gradual worsening with repetitive use can help guide the evaluation. Identifying the location of pain and the activities that trigger symptoms is critical.
